At the top of the form is usually the date, the name of the hospital, the state and the county. Next, you might see the birth announcement, your name, your weight, and the name of the presiding doctor who oversaw the birth.

Finally, you usually the names of your parents and sometimes even your grandparents depending on the state and county you were born in.
