To receive a birth certificate fill out the Louisiana Birth Certificate Mail-in Form. Mail your application to:

Louisiana birth certificates may be received only to the person named, the parents, legal guardians, and other persons who may have a legal right to them for the first 100 years after birth. The fee for the long form birth certificate is $15.50, and the short form is $9.50. You will need to provide identifying information, your photo ID, and fill out appropriate forms.
