The Utah Bureau of Criminal Identification (BCI) operates the criminal record database for the state. They accept in-person requests or mailed requests for criminal records. The fee is $10 payable by credit card, certified check or money order. A request form must be filled out and then taken to local law enforcement for fingerprinting. Afterwards, the completed request may be mailed or remitted in person.

Visit the Utah Public Safety website in order to download the Utah Background Check Request Form.
Step 2:
Fill out the request form and then take it to the closest law enforcement agency to take your fingerprints. Make sure to present government-issued photo identification.

Mail the completed form with a $10 fee paid by credit card, certified check or money order, or drop off the form in person and present photo ID.

Step 1: Visit the Utah Department of Corrections’ website to take advantage of their t Utah Offender Search.
Step 2: Do a search by Offender Number if you know it, or by Offender Name. Hit Search and read and agree to the User Agreement.
