Download the Copy of Vital Record application from the Maine Dept. of Health and Human Services.
Complete the section on the form for a Marriage Record, and include the full name of groom, maiden name of bride, date and place of marriage.
Mail the completed application form to the address below, with a check or money order for $15 for certified copy, or $10 for non-certified copy. Or you may stop by the office in person during business hours. Cash payments are accepted by walk-in orders only.

About Maine Marriage Records
Vital records in the state of Maine are maintained by the Vital Records office, which is a department within the Division of Public Health Systems. All vital records, including marriage records, have been collected and recorded since 1892. However, only records from 1923 to the present are maintained in the Vital Records office. All records from 1892 to 1922 are housed at the Main State Archives.

Fees and Restrictions For Maine Marriage Records
Although there are no restrictions on ordering marriage certificates, any requester must state a legitimate reason for requesting the document. Fees for certified marriage certificates are $15. Non-certified or non-legal copies of marriage certificates are $10. All fees are non-refundable.
